PLEASE FILL OUT ONE PER CHILD AND RETURN COMPLETED PAPERWORK WITH THE FOLLOWING INFORMATION TO THE SCHOOL OFFICE (ALL FORMS CAN BE PICKED UP IN THE MAIN OFFICE):

• Completed registration form (all fields are required)
• Copy of the child’s most recent report card
• Copy of the child’s cumulative records & test scores from previous school
• Copy of the child’s birth certificate
• Copy of the child’s baptismal certificate
• Health History Form Immunization records
• All legal documents of custody (where applicable)
• Proof of Residency (Utility Bill)
• A $50 non-refundable registration fee per family
• Release of Records
• Special Services Form (IEP/SEGO where applicable)
• Extended Day Registration (where applicable)
• Lorain Transportation Form (where applicable)
• Parent Driver's License or State ID 

Ed Choice paperwork MUST be turned into the office, it is not done online. Scholarships (which cover tuition costs) are available to students who would attend the following Lorain City Schools:
